We're happy to answer any questions you may have about the QEII Patient Assistance Grant.
1. How do I apply?
Online applications will be accepted on an ongoing basis by visiting apply.qe2foundation.ca
REGISTER
Locate the QEII Patient Assistance Grant tile. Click More to review the grant criteria. Located on the top right of the page, select LOG IN (if you already have an account) or REGISTER to create a new account. Creating an account allows the applicant to save the application as sections as completed. This only takes a minute! Please use your Nova Scotia Health email address only. Any issues creating an account should be directed to grants@qe2foundation.ca or 902 334 1546.
EMAIL VERIFICATION (New Registrants)
Before the initial application can be completed, a verification email will be sent to the @NSHEALTH email address to finish the account setup.
ONLINE APPLICATION FORM
Once the email account has been verified, the applicant can proceed with the online application. The application will require the following:
- Healthcare provider name, position, and contact information.
- Patient contact information
- Reason for the grant including general income information and area of care (do not include specific health information)
- Upload copies/photos of receipts, equipment quotes or utility bills as backup for the application amount.
2. What is the maximum amount of the grant per patient?
Grants will be awarded up to a maximum of $1,000 per patient per calendar year with a lifetime maximum of $2,000.
- Up to $500 per application for grants to support rent/mortgage, lodging (for healthcare appointments), home utilities (such as power, home heating oil, water bills or phone services), transportation (travel for health care appointments or bus fare) or nutrition support.
- Up to $1,000 for home medical equipment, medical supplies or medication not covered by a current medical plan.
3. Who can apply?
Healthcare providers within Nova Scotia (with a @nshealth email address) are eligible to apply on behalf of a patient of the QEII Health Sciences Centre. Healthcare providers may include social workers, health services managers, physician, medical assistant, nurse, nurse practitioner, pharmacist, therapist, counselor, psychologist, nutritionist, occupational therapist, psychiatrist, or patient navigator. Where possible, please co-ordinate your grant application with other members of your department.
4. What are the criteria for issuing a grant to a patient under the QEII Patient Assistant Grant?
The QEII Foundation will not set any criteria for patient eligibility. We believe that as a healthcare provider, you best understand the needs of your patients. We only ask that you abide by the guidelines as the Foundation has a limited amount of funds available each year.
5. Who Makes Decisions on Grant Applications?
Grants will be reviewed and awarded by the QEII Foundation staff, and all information will be held in strict confidence. Please do not include any personal health information other than area of care or vague financial need information . All applications which meet the criteria of the grant will be awarded until funds are exhausted for the year.
6. When will I be notified of the successful grant?
Applications will be accepted and reviewed on an ongoing basis twice per month. All applicants will be notified via email with the results of the application. Please add grants@qe2foundation.ca to your safe email list to ensure you receive the grant details.
