2. HOW MUCH CAN BE APPLIED FOR IN GIFT CARD VALUE?
Grants will be awarded up to a limit of $1,000 in gift cards of $50 each in the areas of pharmacy, fuel and grocery.
The QEII Foundation will determine the retail partners in each category. There is currently a grant limit of one $1,000 grant per eligible applicant per application period.
3. WHO CAN APPLY?
Healthcare providers employed by Nova Scotia Health who work at the QEII Health Sciences Centre or in a Nova Scotia community-based adult program. Healthcare providers may include and are not limited to: case manager, social worker, nurse, nurse assistant, health service manager, physician, oncologist, therapist, counselor, psychologist, psychiatrist, navigator, dietician, program administrator or physiotherapist. Where possible, please co-ordinate your grant application with other members of your department.
4. WHAT ARE THE CRITERIA FOR ISSUING A PATIENT A GIFT CARD WITHIN PATIENT ESSENTIALS GRANT?
The QEII Foundation will not set any criteria for patient eligibility. We believe that as a healthcare provider you best understand the needs of your patients. We only ask that you abide by the guidelines and complete the impact report and tracking sheet provided with your grant package. You will not be eligible for future grants if you do not submit this information.
5. CAN I ISSUE THESE GIFT CARDS TO ANYONE?
Gift cards may be issued to any eligible patient receiving care at the QEII Health Sciences Centre or in a Nova Scotia community-based adult program. To maintain fairness and avoid conflicts of interest, healthcare providers cannot issue gift cards to their own family members or close personal acquaintances and should request that another provider distribute support in these situations. We recommend a maximum of $100 per patient but distribution is on an as-needed basis as determined by the healthcare provider.
6. WHO MAKES DECISIONS ON GRANT APPLICATIONS?
Grant applications will be reviewed by the QEII Foundation Grant Officer.
7. HOW LONG DO I HAVE TO USE THE GRANT?
Gift cards should be used within six months of receipt of the grant. Should applicants need longer, request an extension by contacting Grants@QE2Foundation.ca. Please note that eligibility to reapply for future grants requires all gift cards previously granted to be distributed to patients with the impact report form and gift card tracking returned to the QEII Foundation.
