ONLINE APPLICATION FORM

Once your account is set up, you will be able to start your online application. The application will require the following:

  • Healthcare provider/applicant contact information
  • Healthcare provider/applicant position at QEII Health Sciences Centre or Nova Scotia Community-based program
  • QEII department or Nova Scotia Community-based program name in which gift cards will be distributed
  • Applicant must read and accept conditions of grant

2. How much can be applied for in gift card value?

Grants will be awarded up to a limit of $500 in gift cards of $25 each or a combination of parking vouchers from our community partners in the areas of personal care, fuel, and grocery and parking (QEII only).

The QEII Foundation will determine the retail partners in each category. There is currently a grant limit of one $500 grant per eligible applicant per application period.

3. Who can apply?

Healthcare providers at the QEII Health Sciences Centre campus or adult Nova Scotia Community-based program locations and employees of NS Health are eligible to apply. Healthcare providers may include and are not limited to: Case Manager, Social Worker, Nurse, Nurse Assistant, Health Service Manager, Physician, Oncologist, Therapist, Counselor, Psychologist or Psychiatrist, Navigator or Program Administrator. Where possible, please co-ordinate your grant application with other members of your department.

4. What are the criteria for issuing a patient a gift card under QEII Patient Essentials Grant?

The QEII Foundation will not set any criteria for patient eligibility. We believe that as a healthcare provider you best understand the needs of your patients. We only ask that you abide by the guidelines and complete the impact report provided with your gift card package. You will not be eligible for future grants if you do not submit the impact report and gift card tracking list.

5. Can I issue these gift cards to anyone?

The gift cards can be issued to any patient of the QEII Health Sciences Centre or Nova Scotia Community-based program. We recommend a maximum of $100 per patient but distribution is on an as-needed basis as determined by the healthcare provider.

6. Who Makes Decisions on Grant Applications?

Grants applications will be reviewed by QEII Foundation staff.

7. How long do I have to use the grant?

Gift cards must be used within 6 months of receipt of the grant. Please note that eligibility to reapply for future grants requires all gift cards previously granted to be distributed to patients with the impact report form and gift card tracking returned to the QEII Foundation.
